Jones who joined Dr. Russell in partnership on the lst January, 1988, and is willing to accept you for the provision of general medical services and unless you wish otherwise your name{s) will be added to his list with effect from the 26th December, 1988, without you having to take any action. If you wish to transfer to the list of a doctor in another practice, you may do so by taking your medical card(s) to the new doctor of your choice by the 20th February, 1989, but if you do not wish your nameisl to be included in either the list of Dr. Jones or any other doctor, please let me know. Requests for home visits should, whenever possible, be made before 10.00 a.m. CONTRACEPTIVE; .5ERVICES Dr. Jones has undertaken to continue the provision of contraceptive service for those patients for whom the service was previously provided by Dr. Russell. Contraceptive service has to be applied for annually and a patient does not necessarily have to be registered with the doctor (or indeed a partner of the doctor) who provides general medical services. if the proposal that Dr. Jones continues to provide the service is not ' acceptable, the Comittee should be advised accordingly by the 20th February, 1989, or alternatively, arrangements made with another doctor.
